try the q&a section but the answer is if you die 1 once or have died ever and you dont have the title, you cant get it. 1 death at any time playing, before or after factions #%@#s up the title.

You should get it at any time, just collect the required amount of xp without dying

You cannot have died, EVER.
You must start at 0 xp, and obtain a survivor title (level 1, 2, or 3, at your discretion) without dying, EVER.
Note: deaths that are "mandatory" (in the Factions Mantid Hatchlings tutorial quest, or the mission(s) where you are automatically killed at the end) do not count towards this.
Your /deaths must return 0 for you to progress towards a survivor title.

No deaths. From the beginning. After you attain a survivor level you keep it, so if you die after making the first level, you will always have it but can't be increased.
Deaths that occur because an NPC dies during a mission, or some other factor, like time runs out, do not count as a death. A "required" death like in one of the Factions mission do not count either.
PVP deaths do count as a death. Even silly festival PvP count. And yes, if you die in the pre-searing PvP on your way to post-searing that counts as well.

Sigh, Naqser, please do some research before answering.
Coridan, to get the survivor title you must reach X exp while having 0 deaths. If die even once, you won't be able to get the title.

That, or your character has to have never died since he/she was created. Basically, if you ever die, the title stops progressing and cannot be restarted - unless you remake the character.
