Offhand vs Shield

How much difference is there with the amount of damage you take with a shield or say a armor +5 health +45 enchanted offhand.... I'm working on a new build and an offhand would be much better than a shield as far as fitting the build which is heavy on energy. I know I could switch weapon sets but i hate that, comments plz
