Most of the KoaBD holders seem to have got 100% explored of both Tyria and Cantha (two points), Protector of Cantha and Tyria (another two points) and Incorrigible Ale-Hound (last point). Legendary Survivor is also not extremely difficult to max, but it obviously excludes characters who've died. The Drunkard title has no such restrictions.

So try for mission completion for both continents and fully explored also. Then you have a few choices for the fifth point.
