WTB: Monk Items with +AL/+Health

I am looking for focus items that require either healing, protection, divine factor, or inspiration that have the following stats.

Armor +5 (While health is above 50%) OR Armor +5 (vs. Physical Damage)
+28 to +30 Health Always

Req. does not matter for healing or protection
Req. preferably 10 or below for divine and inspiration
