Not sure about prices; but yes; the fortitude pommel would replace the pommel of defense.

Perfect Usaly is whith perfect inherent mod on it but some ppl allso add mods like
30+hp and sundering 20/20 in order to get some extra gold..

There's no such thing as a "perfect" weapon, only the best mod for the situation. If, god forbid, you're throwing mending on yourself, a 15% enchanted is much better than a 15 over 50. If you can count on lots of monk backup, a +5 armor is better. If you're suing enchants like shielding and healing hands, a 20% enchant mod might be worth grabbing. "Perfect" is subjective.

perfect is just a term for a particular mod that is the best it can possibly be, so Armour +5 is perfect, +7 vs physical is also Perfect, but to some people they see perfect as a more "high end" term such as 20/20 sundering 15^50 and +30 HP. for example a 20% bonus while health is below 50% is perfect whereas 19% while health is below 50% is not.
