What is 'perfect'?

Well, in all honesty..."perfect" is in the eye of the beholder.
There are only two real criteria, and everything beyond that is subject to interpretation. The most basic necessities are that the weapon be max damage and +15%. Everything else (requirement, +% condition, affixes, etc) is just extra.
Note: Someone will probably reply stating that only a req 7 Sundering Whatever of Fortitude could be considered perfect. S/he is entitled to that opinion; I just choose to disagree.

Usually, Max damage, max damage modifier, req. 9, and a suffux and prefix mod that have max stats. Though, the suffix and prefix mods aren't as important, because they can be overwritten. For example, a Vampiric Gothic Sword of Fortitude with the following stats would be considered perfect:
Slashing Damage: 15-22
Damage +15% (While health is over 50%) - AKA 15^50
Health Regeneration: -1
Life Stealing: 3
Health +30
Slashing Damage: 15-22
Damage +15% (While health is over 50%) - AKA 15^50
Health Regeneration: -1
Life Stealing: 3

IMHO, there's no such thing as a "perfect" weapon for all occasions. However, there are weapons with mod combos that fit certain builds & situations better than others (where "perfect" = best possible weapon for that situation).

In my mind there are 2 variations of perfect which people spam, the wrong one and the right one
the wrong = "WTS Max Dmg Godly Req 7/8/9 20/20 15^50 Great sword Thing +30HP"
Most people wrongly think adding sundering and +30 hp make a weapon godly.
the right description of perfect in my mind is Req 7/8/9 15^50 Max Dmg.
Mods are unimportant because you can change them freely, a cruel prefix and a trollslaying mod dont make a weapon less desirable.
(on a side note, anyone else noticed all the Sephis and Crystalline being sold in anticipation of a NF economy crash ^^ rofl!)

It all what you think is perfect. If you have loads of money you can buy anything you want. If you are poor like me it is the best item you can find. As stated in other posts it depends on builds. Example: If you are a warrior that uses adrenaline skills and relatively no energy skills then a weapon with +15% Damage -5 energy would be the best for you. A warrior that uses a mix of both energy and Adrenaline will prefer a + 15% damage when health is over 50%. Also a perfect weapon changes for your situation Example- Troll Farming / you use enchantments constantly so a + 15% damage while under an enchantment may be the best for you. Under the price check forum there is a thread you can look at the estimated values of rare skins of perfect weapons. Rare skins are no better then cheap skins it is all looks, and collect ability. This is what makes the economics of GW.
You guys forgot staves. 
Perfect staves are, like the others, req8 (Attribute) and either 20/20 HCT/HSR of the required attribute or just generic 10/10 HCT/HSR. As for mods, most people normally demand max HP/MP Mods or matching "Attribute +1 (20%)".
Though most people prefer to buy Green staves to get this perfection.
