Okay, this is just weird.
While running, my character slows to a stop for no apparent reason.
Movement buttons cease to work, though the camera can still be moved.
Warping occurs, but not the typical lag-warp.
I get warped backward about ten feet each time.
Also, several times during combat, everything STOPS. Beasts stop attacking, my guys can't attack, if I'd selected a skill, it just keeps flashing (like it does while you're performing it).....
While in outposts, any chatting I do either doesn't show up, or it takes a year and a half to show.
Adding people to my team is also delayed.
Does anyone have any suggestions?

Sounds like standard, but exteme, lag to me.
I'd start testing out your ISP connection while the problem is happening. Have you tried the -diags flag to have GW perform a network diagnostic?
Copy your GW shortcut and edit it, adding the -diags flag to the end of it, outside of the quotes. You can run this WHILE GW is running normally.
When you start experiencing this sort of lag, run the -diags shortcut. Perhaps it will be of some use.
Another thing you can do is open up a command window before playing, and issue the following command:
This will start tracking ping times to google. Leave this running while playing GW. When you experience lag, check the command window and see if it's having ping timeouts. If not, the problem is probably not your connection. If so, contact your ISP.

Unfortunately the post above is misunderstanding things and definitely not helpful if assumed to be definitive.
The advice to get pingplotter is sound (our support knowledge base advises exactly that), however the IP statement needs clarification:
The IP address provided is only one of many that you may connect to during play. If you're experiencing problems, you will need to know the specific IP and not just one from a commonly used pool (not to mention that IPs will be different depending on which Territory you play on).
6th and 7th hop will vary depending on user. DO NOT ignore 100% packet loss anywhere along the route. It might be normal for some users, but for others it may be indicative of a major problem with an intermediate ISP along the route your data is taking to get to the servers.
Further, the trace you are running does not continue once it reaches the servers. The last few hops are not actually ANet controlled resources, they are our bandwidth provider; a tier1 ISP.
Reasonable advice, though I would not actually advise "complaining" as such. You should, however, contact you ISP support team and enquire about the data you are seeing.
Also very good advice if you have not done so yet. Our support teams are always happy to assist with any problems affecting Guild Wars users. If your problem continues and you do choose to contact support, make sure to include as much info as you can (take some time to look through the Knowledge Base) and be patient - the team will take you through a few steps that may not solve your issue immediately, but will do so if you follow each instruction carefully. The process is based on data that shows us what the most common solutions are, so in the majority of cases problems are solved promptly.

I have similar problems but applicable only to international server. I'm playing on European server and there is no lag when I'm playing there. However, when I switched to international server, at first few seconds everything is smooth but after that, everything frozen. Chat wouldn't come out and sometime I can move. Is there some problem with international server?
Even our guild hall is lagged like the international server. Our guildie which came from the same ISP in Brunei can't do any GvG because of the lag and we assume that GH is using the international server.
I did sent a diagnostic text to the support but seems like the diagnostics failed at pathping.

I usually try to avoid tech-speak, unless I have time to provide screen shots. However, if you want to obtain the current ANET Server that you should test using PingPlotter, run the NETSTAT command.
NETSTAT will show a number of ESTABLISHED connections to plaync servers. The instance server that runs where you're currently at, I believe, is normally the last one in the list. (I think the others are friends server, guild server, and character/account server). Make sure you run NETSTAT while GW is logged in.
As Alex said, pinging will stop once it hits the front of ANET's network. So, you always get a 100 % failure at that point. What you are looking for is a problem inbetween.

I doubt disk fragmentation is the problem, but it should be easy enough to test. When suffering from your extreme lag, is your disk showing a lot of activity? It not, it's not fragmentation.
If it is, it might be fragmentation or it might be something else.
