You either lag out from the mainserver or walk through a certain spot or use assassin skills to teleport yoursefl to your henchmen which can pass a portal without it triggering.

Something similar to this happened durring the trader reset, while the rollback was underway. You could walk a very far way outside of town, while still technicaly being in town. After you got far enough... Somthing similar to that happened.

I remember in Dungeon Siege that sometimes map-loading went wrong, particularly if a pixie had recently zapped you with some extra health - you'd end up at the edge of the world, trying to remember how long ago you last did a quicksave!

I really wish they didnt take away the portal bug at the citadel. We used to hold Hide and Seek matches outside the camp, good fun.

It's not a bug or a glitch.
The game only renders so much area when you are in a mission and an explorable area. The area is usually based on how far you can see from certain points in the area.
Rendering the entire world would require so much memory and so much to load that the requirements for the game would be much higher than it is now... I doubt it would even be playable if it tried to render the entire world map.
Because it only renders so much, it basically does what you see in the above pictures. If you pay attention, in any mission or explorable area, there is a point at the edges that you can see the rendered area end in the radar just like in the images above. Usually the location is behind a large mountain range or something, you can't get to it, but there are a few small places you can get right to it.
What basically happened to you is a form of lag, you went for a portal to another area but it didn't "zone" you to that area, you just kept running and eventually got to the limit of the rendered area. There are some portals that were glitched in the sense that you would be able to sneak around the portal that would zone you, and just keep going. Two of these glitched portals, which I believe have been fixed, were Tombs (back when it was PvP) into Dragon Lair, and Granite Citadel into Tasca's Demise.
Anyway, some of you seemed confused as to why this was happening and no one was posting an explaination, so I thought I would give it a try. I just hope it said it in a way that makes sense.

i think its a lag thing becuase it happened to me a couple of times where ui was able to run all the way to another town (it didnt cut me off like some of you guys lol) - i couldn't getb though the portal though

In this you will notice two things. One I suck at panorama's, terribly. Two, the dragon festival had one of the best of this type of glitch ever.
I reiterate, I know I suck at putting shots together, but they weren't taken to be put together, and I suck at it in general. However, it gets the point across imo:-p

I have a shot of this as well....in one of the northern shiverpeak missions....thought it was funny so I took a pic.........

That's called a "black wall" in computer games. All of the game's models and visuals are stored on your computer, but ask the servers when to load that information. Sometimes, when your connection lags, your computer refuses to load the models and walkmesh of areas ourside your view range from when you disconnected, creating a blank wall around the area.
A more rare occurrance of this is when the servers for whatever reason do not acknowledge that you've gone through a portal, so it allows you to walk behind the portal to the end of the area. This is a random bug that cannot be fixed. It's happened to me only once in my 16 months of having GW.

I think it was the second Dragon Festival Mission. I simply ran backwards, and lo and behold, none of the closed gates where stoping me ^_^

There was a glitch like this for Granite Citadel where you walk through the portal by hugging the left wall.

Oh yes, one of the cleverer ways to get another 0.1% of the map cleared. It does look strange and under-lit up there, though the only screenie I managed to take when I was up there just shows a lack of effort on the texturing...not really the same as "running out of ground":
