I've seen and heard both stories on this, which one is right...

1) The only stats for an IDS are:
* Blue Rarity
* Cold damage: 15-22 (req. 9 Swordsmanship)
* Damage +15% (while Health is above 50%)

2) Or that they have variable drops like other weapons, different colors, etc.

Icy Dragon Swords are only dropped in what you described in choice #1.

However, you made a slight mistake.
All Icy Dragon Swords are +15% damage while enchanted, not +15% damage while above 50%.

The OP is correct, Hawk is mistaken.

They are 15% over 50% health.
