In order to reach the uw or fow lets say in Tyria do i need to ascend in tyria if i have allready ascended in Nightfall or any other campaign.

no, I't doesn't matter what campaign you ascend in. If you ascend in cantha,you can still go to UW throguh ToA.

What is considered Ascending in Nightfall? (I haven't gotten very far in the story yet, just did the first mission in Vabbi, so if it's further down the road, no spoilers please

well if its true then why is monk not able to access ToA anymore its canthan accended but wont allow me to use ToA any more?

No it isn't, as soon as I got my Paragon to Chantry of Secrets just following the storyline, I was able to get into FoW. I then went to ToA with the same character and was able to get into FoW from there.
I have yet to do anything involving wurms, the farthest I am is Jennur's Horde (Spelling)

Maybe ANet decided it was getting too repetitive? Who knows.

Usually being able to change secondary profession indicates ascension. So I suspect it's related to Sunspear rank and then being able to change your secondary profession.
