Maybe I am missing something. I am in the realm of torment, all the way up to the Gate of madness mission. Can anyone tell me how to get to Shadow Nexus? is it an exit from an existing town or do I have to go out and find it like the Gates of nightfallen Jahai?

It's an exit from the first outpost in Realm of Torment - Gates of Torment, if I recall.
