Blacktide Den death giltch?

I'm going for survivor titles with my Dervish. I used my Tyrian ranger to scope out the Jokurnar Diggings mission and it seems fine. The crusher traps don't count towards the death count.

However, I just spoke to a level 20 Dervish who told me about a glitch or something in the Blacktide mission. He said that he never died as in his health going to zero. But at some point between the cutscenes, the game thought he died and got 1 on his death count. He said something about the corsair garb, but I didn't quite get what he was saying.

Anyways, is there a known glitch that causes a death count to go up by one? Or could the person I spoke with just suffered from a random glitch that could happen anywhere at any time? Does anyone have a survivor Dervish or any NF character that is way past the Blacktide mission?

EDIT: I'm speaking with him and he said it must have happened between halfway thru the quest to gain entrance(which I suppose is "Following the Trail") and putting on the corsair garb in the mission. He said he never resigned, mapped out, logged out, or closed GW during this timespan he described.

He probably just died once and won't own up to it.

It's very easy to get Survivor in Elona as long as you play smart. I would have had survivor on my Dervish, had my friend not taken that Master quest with the Skale onslaught when I was only level 6.

After that lonely death (which was an outrage at the time, too lazy to remake my character ), I didn't die again until going to cap Avatar of Balthazar. Post Level 20.

Okay cool, so you have one death from that and none from the Blacktide mission or the quest right before it. That's good news to me. All I need is to get to the point where I can go to Lions Arch. And from there, Cantha. Once there, I'm guaranteed the legendary title.

I'm still on the Leaving a Legacy quest and I'm almost level 11. I'll just have a friend bring a max tank to help me out. Any other advice?
