Doe a single Insignia covers all the armor pieces , or just the one it is placed on ?

It only covers the piece it's put on.

It only covers the piece of armour it's placed on. If you place it on your chest, only that gets the bonus. So if you only have one right now, I'd stick it on the chest, since that's the part that is attacked most often.
