Axe is Sephis Axe. Sword would be called the Eternal Sword and doesn't exist in game (yet).
And I know why you're asking this...

every time i see the ghostly hero in heros ascent now, he is weilding a chaos axe.

Yes, an update about a month ago changed him from weilding a Sephis Axe to wielding a Chaos Axe, This was just done so his axe choice fit better with the "ghostly nature" of his other weapons.

There was a time when he didn't even have an axe, he would just Cyclone Axe with his sword lol.

The sword, i think but not sure, was able to be used in beginning of the game, unsure but they seem to be rare beyond rare
