I'm playing Prophecies--Game of the Year at this point but I'm in Lion's Arch (not Gate). I understand that Lion's Gate includes teleports to start Factions and Nightfall from? I think I want to finish all of Prophecies before I start any of Factions or Nightfall and I've only gone this far in Prophecies, i.e., there is a lot more to Porphecies to play out still--only 7/25 missions completed. If (not using the word "install") I purchase and add the key codes for Factions and Nightfall, will those downloads add or change or enhance any content of Prophacies--Game of the Year while I continue to play it out?

No, adding factions and nightfall to your prophecies account will have no effect on the gameplay in prophecies. It will just give you the option to travel to cantha (factions) or elona (nightfall) from Lions Arch.

One thing it will give you is the option to travel to Cantha(Factions) and/or Elona(Nightfall) from Lion's Arch to get the maximum value armor earlier than normally in the Prophecies Campaign, and some more skills (if you have the gold to buy them). And even if you just get Nightfall you can get heroes to help with some of the missions that have mostly empty towns, including Olias the Necromancer from Lion's Arch. But otherwise they won't enhance the content as such.

I added Nightfall once I reached LA to get the Heroes. Also I was level 18 then and used the high-xp Nightfall quests to reach level 20

Adding Nightfall will change a lot of things in Prophecies. Well, okay, maybe not a lot of things, but it will change the game in perspective for you.
Nightfall brings Sun Spear quest and Recruit Olias. If you also add Factions, you get to have access to all of the non-elite skills in Factions once you complete problems in Cantha. And with both together you gain the quest in Cantha, recruit Zamai (the Assassin). Both heros and the ability to purchase many skills (and get maxed out armor in the main city of Factions - which is the first city you arrive in Factions), will change the challenges of Prophecies.
Does not change the story, the graphics, or people's attitude towards you, but it makes Prophecies much easier.

In terms of lore Profecies, Factions and Nightfall work as a trilogy. One complements the other, althought the connection isn't quite visible until the end of nightfall. I will not ruin the story now delivering a spoilers galore but what I want you to know is that, if you want to really follow the plotline you should go by that order: proph, factions, nf, because they are in chronological order.
If I were you, I would merge the accounts now, and only start playing when you got over prophecies, then you would move onto factions and the nightfall, and you would see most of your story related doubts answered.
Don't worry if you merge the accounts, it will not change the story or anything. Though, I wouldn't move to elona, searching for heroes because to do that you will have to reveal parts of the story that you are not ready to reveal yet, since you haven't completed prophs.
Oh, and it would be better if you stopped calling it "Prophecies -- game of the year". We all know that already! (factions is also game of the year)
