Sword with two damage mods?

My brother found a sword with two damage mods, they are:
Damage +15%(enchanted)
Damage +15%(while hexed)

It is a blue sword. I thought maybe he got it from a quest reward but I looked it up and could not find anything. He also says he thinks he found it and has not done any quests recently. To my knowledge it is not possible for any weapon to spawn with two damage mods, but I may be wrong. Could you guys help me out here? Hopefully I'm not a noob on this

That should be a pre-searing quest reward.

I don't think it is pre-searing because the stats seem a tad bit high. He also says that he has deleted all the items from pre-searing and I don't remember that being in his inventory before.

damage 10-14 req 3 sword.

you can find 15^50 mods low req weapons.

It's not a photoshop, I could show in game if necessary. I checked all pre-searing quest rewards and it is not one of them. I know that you can't find 15% damage mods on weapons that low, that is also why I am curious.

any low req blue sword with perf mods is obviously a quest reward.. thats all it is, no more no less have fun with the quest reward

A quote from guildwiki "Axes, Bows, Hammers, Swords, Daggers, Scythes, and Spears may have at most one modifier." it shows no exceptions to the rule as in a quest or collector. This is still confusing to me.

theres plenty of lower level quests that many people havn't bothered to add onto Wiki because they're so trivial... and besides, quest items from PRe and Ascalon area are useless with or without special modifiers

That's a quest reward from Pre-Searing. Used to have an inherit +5 armor on it instead of one of the 15^* iirc.
