You get the quest (Poor Tenant) from Namar in pre-searing Ascalon City. It is the one where you have to go see Miller Upton then lure the swarms of bees across a bridge. Most people ditch this offhand but its very useful to keep for later in the game.

Why not buy a cheap purple/gold one? If I remember correctly that pre-sear offhand has less energy then a purple/gold would. It's been a while since I've seen the pre-sear item so I could be wrong.

Also there are -20 health inscriptions that you can put on any inscribable offhand you want to.
