Do quests, but always clear the way the quest takes you. The quest rewards plus the kills will yield nice XP, and you will have most fun this way, as it won't feel like grind. Other techniques may be faster, but trust me - even if you take levelling slowly, you'll reach lvl 20 soon enough.

The game only really starts at lvl 20. Essentially, your group of 8 will be fighting groups of enemies of levels 20-28. Actually, the feeling will not be so much different from what it feels now. However, the real challenge is to adapt your character to the situation at hand. One mission will require heavy firepower, favouring elementalists, while another will need to shut down enemies, making mesmer allies a necessity. Also, the game will become more demanding concept-wise. Your brute force will be limited, and you will need to learn how to interrupt the enemies when they use the most dangerous skills and how do properly isolate groups of enemies (You won't get past gates of Pain otherwise

). The importance of breaking stances, removing enchantments and similar also gain in importance. Ideally, you'll switch your skills after every battle to adapt to the new situation.

It is a common choice, usually considered the newbie combo. However, this just reflects that the choice is very common. Any class combination can make for a good and effective character. But don't worry - soon, you'll reach the point when you can change your secondary class at will. Remember: at first, some skills will work (like "Mending"), but later in the game, you'll have to abandon them for others that require more skill to use, but have greater effects.

OK, nice and slow, it'll go. First, your Prophecies and Nightfall accounts need to be "merged" - one account, six character slots, not two accounts with four slots each.
After that, you can travel from one continent to another. First, the travel needs to be unlocked by completing a certain quest. The quests are not immediately available.
For a prophecies character, you'll need to reach the city of Lion's arch. There, you will find the quest giver for the quest that will unlock travel to Elona. Note that you may need some human help, as the quest will have you facing some high lvl foes, and the Lion's arch henchmen are low level and of little use there. After the quest is complete, you will be taken to Kamadan. From then on, you will be able to travel between the continents simply by clicking the boat icon on the map screen and selecting the target continent.
Note that some parts of each chapter are limited to native characters only (the tutorial areas and quests, and also some of the quests), so it usually pays to play through a campaign with a native character at least once.

Each class has a maximum armor level (60-80). The armor level determines how much damage you take from a hit/spell (unlike for example Diablo II, attacks always hit, but the amount of damage differs radically). The system is a bit complex, but the base rule of thumb is: armor level 60 means you take as much damage as the skill icon shows (100% damage), and increasing/decreasing armor by 40 will result in halving/doubling the damage taken.
Also note that different armors work differently against different damage types. For example, a paragon and a warrior both have basic armor of 80. However, the warrior's armor will also always have an additional 20 armor against physical damage, allowing the warrior to survive melee much better than a paragon. Similarly, a ranger may have armor 70, but also gets a bonus of 30 to elemental damage (meaning that he will be tougher to usually ranged elemental damage than a warrior, but still significantly weaker against melee).
You will soon find out that these things make a hell of a lot of a difference in the game. Characters with armor 60 have nothing to do in melee but die

Depends on taste, but I would suggest first playing through all with one character, and then trying the other one to see the parts that you missed. It's not much, but it does make for a different experience. The native character will get the whole story, while the foreign character will find some of the things already done.
