Random Deaths While Going For Survivor?

I am trying to go for the survivor title but I can't reach my goal due to random deaths. While playing the game I have encountered random deaths many times. After completing Vizunah Square it said I died when I didn't, during the whole mission I stayed far back and never aggroed anything.

did you have a Minion master on your party?
did your party had minions during the ending cinematic?
dir your party watch the entire cinema until your team was killed and the minions went berserk?

if the answer to the 3 questions above is yes, then you got struck by a bug as you were "Killed" by the minions during the cinema.

if you re-try with another survivor, dont get a MM or ask your team to skip the cinema and it shouldnt happen.

all in all, bad luck buddy.

Your character has to die in order for you to receive a death.
