Was doing a quest from Ascalon city. It was an artifact quest to ser.Temple. Now if you look at the map,This place is just to the right of Ascalon city. But the route was a long way around and south.
So i got the henchman in Ascalon city. they are lvl 3. The route had me going through some higher lvl places with mobs at lvl 5 to 9. The question is do henchman lvl with the zones you go in? I don't think they do as they all died on the way really quick.
Also for some reason, I was missing alot in combat. I've missed before but not as bad as this. Did something change in a patch? I was missing like five times in a row. Almost got beat down by lvl 5 mobs and i'm lvl 13.

No, henchmen don't level up or down per zone. They are, however, dependent upon which outpost you decide to start from. Ideally for that quest you'd exit from Frontier Gate, which is reached after completing the Fort Ranik mission.
As for the missing attacks, I'd be willing to wager that you're encountering the Stone Elementals who cast Eruption. You don't normally miss attacks unless there's a hex or condition that causes it.

And you were right about the mob i was missing hits on. How can i tell what skills mobs have?

you can see what skill they're using if you have them selected. hexes and conditions on you show up on your screen (default location is upper left corner of your screen).
henchie level just depends on the most recent outpost you were in. you don't have to start a quest from the same area you received it in. there's quite a few quests that its just closer and faster to start from a different town.
