Well, er, why aren't you taking mystic regeneration instead of healing breeze and mending? I mean, wouldn't that be the logical choice? Have you decided on an elite? You'll have to give us a bit more to go on. Edit: Even as a low level, you should be planning ahead. Remember you can buy skills with balth faction and then buy them from a trainer in PvE once they are unlocked.

There are the avatars to choose from and after that, reaper's sweep and onslaught happen to be the most popular. Some also choose ebon dust aura. You'll want an IAS as well in most cases which means heart of fury in most cases as most people don't use whirling charge. Consider switching your sub to warrior and using flail or so. You'll only get 4 seconds but with your attack skill spam, you should be able to keep it up indefinitely as you need it even without a furious mod or enraging charge. Twin moon sweep isn't all that great - consider victorious sweep instead. Also, your attack chain should go something like victorious -> mystic -> chilling/reapers sweep/whatever. Lyssa's assault is also great if you use onslaught as your elite. If you use, say Melandru as your avatar, you'll definitely want wearying strike for deep wound on demand. Mystic vigor is kind of meh in my opinion but may work with Avatar of Dwayna. Consider conviction or a cover enchant or simply another enchant for mystic regeneration.

Edit: Reaper's sweep isn't too far into nightfall. Neither are the avatars so you should think about which route you want to go. Personally, for PvE, I'd choose reaper's sweep in most scenarios simply because I can mindlessly click buttons and kill stuff.

i personally think u shouldnt just go dervish skills...

try using elementalist skills to your advantage (conjure frost, fire etc.) and change ur wep to a icy of firey wep. chilling vic is probably the most effective as ur able to do extra dmg wen fighting mobs. mystic regen is a must and conviction and VB u should keep as they hlp wth ur overall stats. for elites wounding or reaper seem like the better choise.

my advice, dont use wind prayers as their not as helpfull as earth prayers and scythe.

Actually, wind prayers are great for PvP at least but since this seems more about PvE, I'll leave off on that.

While scythes are big damage and with conjure, even larger damage plus it counts as an enchant, I don't think you can do without things like wild blow or distracting blow in Nightfall but I suppose you can always stick those on heroes.

Well Dervishes don't really function that well at a low level due to a low mysticism attribute and thus a lack of energy.

I'd take Heart of Fury or Flail as you need an IAS with a slow weapon like the scythe.

Your elite can be either of the scythe elites for Deep Wound, or if Deep Wound is already covered in your party then Zealous Vow can be used to spam scythe attacks continuously. Avatars are generally useless for an elite; some of them are good for PvP but for PvE they are generally not as useful.

Also Hand of the Forgotten sucks. Vamp > Sundering for damage in PvE, 15^50 isn't as great as 15^enchant because you should always be enchanted in PvE, and +30hp isn't that great on melee characters. For a Dervish +20% enchanting is probably ideal.
