Moving throughout zones

I'm having trouble finding a good guide that explains how you move from one zone to the next. I've reached many of them by doing quests and missions but I'm no stuck as to how to move to some of the uber zones. Is there a guide out there somewhere that can explain how these zones tie together?

Press 'M' and click on the area you want to go? This is for outpost zoning.

If you are talking about map zone in exploration, then you have to find the white portal that will zone you to another area.
