Hero's Pet is Level 17 but never evolved at all?

I tamed a phoenix with Magrid and levelled it from level 5 to level 17 but it has not evolved at all so far.

It was dead the whole time I levelled up since it died automatically when I entered a worm.

My question is: will this pet ever evolve, or is it permanently fubared?

If it will evolve, at what point would it evolve?

It may be that the Pheonix won't evolve. I know it doesn't grow as it levels up but not sure about evolution since I named mine as soon as I got it. Is there a way to check this? This web page doesn't address your question but it may help some.

Pet evolution
Generally, the pet evolves at levels 11 and 15. However, in certain cases this has varied; such instances are rare and not fully understood, though leveling up while dead does seem to delay evolution to the next time the pet levels up. Other times the pet name does not update appropriately; in this case, name your pet, then reset the default name with /namepet or /petname. Which evolutionary path the pet takes is dependent on the play style of its master leading up to the pet's evolution and is still undergoing research.

If the pet leveled every time it was dead maybe it's just waiting to do so while alive.

I'll assume you know to /petname reset, and if that doesn't fix it.... My guess is that you levelled while SS farming in hard mode? generally my experience with pets and evolving is they will very often *not evolve* if you take them straight to 20+ level foes which kill them instantly, or even when they still deal all of the damage. I had done several phoenixes on the outcasts in gyala (the ones fighting luxon guards) using a standard beast mastery training build which evolved improperly or not at all. I got one level 20 aggressive phoenix and one non evolved out of 4 (the other two still went dire). I don't know why this is, but it's what I've noticed. I suspect it doesn't matter all that much whether the pet is dead or not, you'll always have screwy evolution in hard mode since opponent level has been the only consistent factor in pet evolution that I've ever seen.
