Arthas-90
hmm I Took Mesmer with domination (to kick shamans), fire ele, dunkoro. And it was very easy.
Freeman
I used Olias as MM, Zhed as star burster, and Aurora as interrupt (I was monk) so you could sub aurora as dunkoro or tahlkora.
Dinkytowner
If you're a ranger, go with BHA + apply poison, a SF spammer, unsteady ground/Ward mele ele, and a ZB or Life Sheath Prot monk. I use a build similar to this almost everywhere I go to vanquish and it rolls most everything.

For the early 4 man missions I believe I used monk, curses necro, warrior, b surge ele. You can do it with other builds of course, but the key is reducing the incoming damage. Your hero monk probably won't respond well to being backbreaker chained by grawl if you're not doing something to help him out. If you're playing ranger, you'll have a hard time interruping without BHA because of the half cast times. I'd probably go BHA to neutralize a monk or caster, and take throw dirt or something to help keep the melee in check.
As for HM in general, I found myself changing builds a lot, depending on what was dominant in certain missions. I don't know if there is one great build that will get you past everything. For example I just did Gate of Pain in hm, and really the only thing dangerous was the fire eles. I had to build almost entirely to shutdown and minimize damage, whereas if I'd had a bunch of melee shutdown or a MM, I'd probably still be failing it.
