Vanquishing title some questions

What exactly do you have to do to get this?

And how many areas are there in prophecies?

Lastly do you just explore and kill (my understanding of vanquishing) or do you have to redo every mission as well?

1) Kill every monster in every exploreable area on Hard Mode. Of Course you do one area at a time.

2) I think about 54.

3) No just explore and kill, there is a separate title track ( [wiki]Guardian (title)[/wiki] ) for doing every mission on hard mode.

54 is correct. Some tips for Vanquishing: Bring an MM or 2, they make it so much easier and make sure you balance out healing and damage to something that works, I don't know how many times I've gotten down to the last couple groups only to die because we didn't have enough damage to take down a boss or something.

Go for the tough parts on the map first. (ie. Bosses, big mobs)

That way you'll know if you can handle the zone.
Or if need be, kill some easier foes in the area to work off some DP.

One thing to note, you don't have to have every mob in a zone dead, you simply have to have killed every mob. What I mean by this is in some zones in the game, you'll run into mobs that res their buddies (especially prevalent in NF and Factions), once you've killed a mob, even if it's then res'd, it counts as done so far as vanquishing goes.
