I had an intersting experience with Cantha trying to work out why the game was only letting me have 2 hero's instead of 3 in a party. I only had 2 in the party and I couldn't load a third. I could take any of the two out and reload back in a 2nd, but I could not add a 3rd at all.
It then dawned on a guildie that I had finished the preview event with Vekk loaded and even though he was not showing as added he was unavailable and taking up the 3rd place.
Changing locations or reloading the game wasn't changing anything till a different guildie suggested an effective fix.
You can map your character to a single player quest location - ie one where the game forces you to lose all heroes.
In Cantha the solution was to map to Fort Aspenwood. I imagine the things you can do in the other campaigns is do the solo missions (ie Thiark's Orchard or worm challenge in Elona or the Doppelganger in Tyria).
While many way well have caught on faster than me, I am posting this here because I am sure I won't be the only one that was puzzled out there about this.
Edit - I can confirm that 2 of those 3 other solutions don't work. The Orchard requires you to drop all from your party (which you can't do with one hidden hero) and the worm challenge was not effective so it must give you your hero's back afterwards. I was able to remove the hidden hero with simply zoning into the mesa area at the start of the doppleganger mission.
Something to watch out for if you left a GWEN hero in party

I had the same issue as the OP...mapped from Beetletun to Fort Ranik and I still couldn't add a third. Then I mapped to Kamadan...problem solved. Just switching continents worked for me.

Best solution is to map-travel to Random Arenas outpost.

Random areanas makes sense in that it is the same - single person only. I didn't think of that because I have never actually pvp'ed there .
On the subject of continent changing, that one doesn;t make sense so I am unsure of that. I did that when on my 2nd character I tried to get an elona fix. I am pretty sure it was still an issue after I had mapped to Tyria, but not after I had gone down to to mesa (but I may have been hasty in not checking properly).
