Get the area map, it makes it a lot easier. Simplest route is to open the gates of strength, and then go left and follow the curve around until you reach several chilling wisps in a circle, kill those, and there should be a rez shrine by a proof of kinship. Then reverse your path, as you will have gone by a gate of kinship, inside of which is a proof of duty, then go back on your original path, and keep going in a straight line and you'll reach the portal eventually

I got the map and I've cleared everything [I think, might have missed a few] and so I'm pretty confused.

D: Help, anyone?

I'd think it would be more straightforward than it seems to me, perhaps it's a glitch?
