Near Tydus in old ascalon there's a guild emblem npc that will make one for 2k.
Theres also one in shing jea monestary and Kamdan.

In Kamadan, there is a Guild Emblemer NPC near the portal to either Consulate or Sun Docks. I don't entirely remember off the top of my head.

And... you wouldn't happen to watch Frasier, would you?
