How many possible mods can you have on a wand or focus item maximum?

You can have one inscription and one wand wrapping/focus core. That's the "of XYZ" part of the weapon name. Applying a new mod of either type will only replace whatever is currently being used.

in short 2 mods per weapon or offhand. Go to the priest of balthazar for a full list of all possible.

Note to add an inscription the item must either already have an inscription or have listed "inscription : none"

No, it doesn't. Shields, offhands, and wands don't get a prefix. I already answered his/her question, so please refrain from spreading misinformation.

No it doesn't go for wands or offhands it applies to staffs and melee weapons and bows.

This is only true for Swords, Axes, Hamers, Bows, Scythes, Spears and Staffs.

All other items can only have 2 mods attached to them.
