Question about Eternal Conqueror of Sorrow's Furnace & Slaver's Exile HM

Ok, I have 2 questions.

1. To get Eternal Conqueror of Sorrow's Furnace in your Hall of Monuments, you only need to complete the Final Assault quest? Or also the To Sorrow's Furnace quests (I've done all of these before Eye of the North's release).

2. To get credit for Slaver's Exile in your Master Dungeon Guide [Hard Mode], do you have to beat all bosses on Hard Mode, or can you just do the first ones on Normal Mode, and only kill Duncan the Black on Hard Mode?

1. You need to have Final Assault quest yourself and to finish it after GWEN release of that item in HoM came out, sort of thing.

2. You just need to kill duncan in HM

I killed duncan in HM, got it logged in my book, but havent got the option to display it
