I was looking up some info on skills and was reading up about the primary attribute of the classes, and found this:
"All skills are shown with minimum and maximum values. The minimum is with 0 points in the related attribute; the maximum is with 15 points in the related attribute. It is possible to have more than 15 points in a given attribute, but values will need to be calculated on the 15-point figures given in the individual descriptions."
reasons i ask is I always have 16 attribute when I have a +1+3 head gear ...
is this a nerf or thats a typing/information error or I am making the reading comprehension mistake again?

Reading comprehension. When you look at the skill effect and it has X...XX, that maximum number is with 15 attribute. Higher than that and it's calculated in-game. I do notice differences in skill results with >15 attribute.

No. What it means is that the scaling is done from 0..15 attributes. Anything in between 0 and 15, or above 15, is extrapolated. It's why very, very many skills have "nice" numbers like 5..20 seconds (Withering Aura) or 30..120 dmg (HEV), or why you get funky scaling on ranges like 8..20 (Spear of Lightning) or 20..70 (Orison of Healing). In between 0 and 15, the game rounds; at 0 and 15, you are guaranteed whole numbers.
