Create a shortcut to Guild Wars on your desktop.
Right click on the shortcut and go to properties.
Under "Target" it should say somehting like "C:\Program Files\Guild Wars\Gw.exe"
Add [space]-image to the end so it will look like
"C:\Program Files\Guild Wars\Gw.exe" -image
Double click the shortcut.
GW will download all the files and junk you don't have then it will close automatically.
Remove -image from the end of the Target line and play GW.
If you don't take off -image it won't open.
EDIT: If you have vista it's a slightly different process and theres a guide for it, just search for "image" and "vista".

or an easier way, you don't even need to mess about with the shortcut.
Start>Run>"C:\Program Files\Guild Wars\Gw.exe" -image>Ok
Start>Run>"C:\Program Files\Guild Wars\Gw.exe" -image>Ok
