You and Alesia are the last ones of your H/H team. You run away and break aggro. Both are at a sliver of health and Alesia is diseased as well. Alesia runs up to you, infects you and dies. Before you can finish casting your res on her you die from the degen.
Besides that I like Alesia.

In pve - In times Before Wikied Nightfall, loosing a 600k survivor when you press ,,Don't Touch!'' in Vabbi without pre-proting. It was kinda amusing - Searing Flames, Searing Flames -260, Searing Flames -250.... 5 seconds later everyone died. And ,,You have died 1 time''.
In pvp - Almost winning HoH, then a Blood Spike comes to you and spikes half of your team in 2 seconds (Enfeebling Blood + Oppressive Gaze = thank-god-they-nerfed-it). Boom, you lost by 1 point.

Wacky
Taking almost 2 hours to get all the way to the end of Kathandrax (or any other dungeon) without any difficulties, then finding that he or the rolling balls slaughters your H/H party and there's just no way with your party's skillset to kill him.

Walking into the desolation, for some unknown reason not realising the sand's gone bright yellow and dying half a second later.. doesn't cause the death count to go up though.. which is nice given I'm slowly progressing the character after finishing the survivor title without dying and without fear of dying now.

Worse death ever. Getting your ranger to lvl 2 survivor by doing it the right way questing and missions. Making it to hells precipice and having some moron aggro a whole friggin army of fire imps who attack at once and cause A massive lag. Never able to move = Dead Survivor

Near whipe was caused by two W/any running to the bonus, effectivly killing themselves, the seer and 4 other teammates.
As for the worst way to die. I'd say after spending 3 hours on any map and right before the finish is always a bad time to die.
And for all you wammo's out there, if I'm monking and you leave my reach you are on your own.

You're clearing fow and just done the quest near the seige wurms, the mage comes along and opens the gate, you complete the quest, you only have 3 left to do, all runs off but you as the NCP mage is trying to solo a seige wurm, you start begging your team mates to come back and help him out as you know if he dies thats it....they all disagree with you and say he doesnt need to live...you have no choice to follow. 30 secs later we're all dead....

Ok, I can beat all of you with this true story. Last weekend our guild planned a "clear FOW" event. Due to start at 8pm. Well, the guy planning the event was unable to make it due to a freaky internet connection. The rest of us formed up, but it was too disorganized and we broke up and did other stuff.
Well we really did want to go to the FOW and there seemed to be enough interest on Alliance chat so we tried again around 10pm. We formed up a group of mainly newbs. 6 of the 8 people admitted they had never been in FOW before or had just "peeked their heads in" and gone no farther than the first Tower of Courage quest.
We decide to go for it with the intention of not clearing, but just giving the newbies a peek at the place. Our party make up was not ideal. We had about 6 Rangers, 1 monk and 1 ele.
Before starting we convinced most of the rangers to switch to other chars.
Next we end up running those other chars to ToA.
10:30pm. We finally warp in with 2 rangers, 2 eles, 1 mesmer (who we find out later was really a ranger in disguise), 2 monks and 1 poor Tank that we decide to shove in front of every mob.
We clear the tower of courage. Pretty easily I would say.
Next we start killing random monsters near the tower because this group really has no direction and no goals.
We move on to the Battlefield and kill most monsters there.
Complete the Army of Darkness and move on to the forge.
Clear the forge. Pick up all the "forge quests"
Complete Restoring the forge.
10 minute merching break!!!
It's now 11:30 and our tank has to leave.
Ok, now what? We have no tank. We decide to move on to The Tower of Strength and Kobay and see how far we get with no tank.
Turns out that 3 Barrage rangers and 2 Fire Nukers really don't need a tank. We demolished most mobs with only a few problems.
Ok, 7/11 quests done. Not bad for a group of newbs. Let's move into the forest and complete Wailing Lord.
Yikes!!!! That forest was tough. We goofed badly and our monk and 1 Ranger were the only ones left standing. The rest of us were grouped around the Wailing Lord and a bunch of skeletals way out of range of a safe rebirth.
Our monk and ranger really rallied together and managed to kill/distract enough bad guys to safely get in and rez the rest of us. Time wasted: a half hour.
Griffins are next. (Not a quest to attempt with a bunch of fow virgins!!!)
We convince (sat on and forced) the mesmer to stay behind with the griffins. The rest of us journey back to Rastigan clearing the way.
Very close call at the end, but the griffins arrived safely.
9/11 done. Wow! Never expected this. Who knew our newbs were so good? Talk now turns to the end chest and the riches that await us.
Burning forest is next. None of us have ever been in this area before and have no clue what to expect. We journeyed over there and got the quest. We found a BUNCH!!! of heavy mobs. Our rangers expertly pulled the key targets away from the mobs and we complete the quest without much bloodshed on either side.
Last quest! The wolves. Otherwise known as the grand tour. We get the quest with a few mishaps. Seems some people were too eager to reach the quest giver and they ignored the 10-20 Shadow Beast pop-ups.
It's now 2am. We just want to go to bed! (But not before raiding that end chest and getting the statue in HoM.)
We kill the 1st wolf. No problem.

3 wolves
4 wolves
5 wolves
6 wolves Woohoooo!!!!!!
1 last wolf left on the beach. Someplace we haven't been yet. Well we're already near the tower of courage so we start towards the beach and encounter a lot of pop-ups near the tower that somehow we previously missed.
We panicked and managed to run in 5 different directions to escape the mob. I ended up near the spawn point with a monk.
3 of us were dead.
1 other was near the tower
and 2 others were someplace else.
We yell at everyone to stop aggroing and regroup at the tower.
When someone starts screaming. THEY'RE IN THE TOWER!!!!!! THEY'RE IN THE TOWER!!!! RASTIGAN'S GONNA DIE!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
The monk and I kinda blew him off since Rastigan is in our Ally list and his health bar is perfectly fine. We tell him to calm down. We're fine. (But we're hightailing it back to the tower anyways!)
All of a sudden we wipe. Whole party dead! Turns out the mob got one of the griffins. I always thought they were safe once the Griffin quest was complete.
10/11 quests done and the final quest 6/7 done. And it's 2:30am.
I can't think of a worse death than that. Can you?

In my opinion, the worst way to die is when absolutely nothing is going right, your opponent has the perfect counter to whatever you're running and the players on your team are doing poorly anyway, yet it still takes 5+ minutes for everyone to actually die. Slow, painful health degeneration as you frustratedly consider screaming at Vent.

Worst way to die: Playing a 1hp BiP in HM Foundry, party whipes in the last room except for you. You use res sig on the rebirth monk..but die while walking over to res him because you forgot to swap to high health armor..

I have a similar story to the FoW incident. Our guild got together last weekend to clear FoW. We were doing great, had a great leader, clearing everything slowly and doing all of the quests. The leader of our group had attempted this a week or so before and Anet booted her out or lost connection (not sure which) with 9/11 quests complete.
It gets worse..... So as said before, everything is going great, and we finish the 9th of the 11 quests and start on the 10th one. We clear the hydras, and the burning forest, ever so carefully, and have only 3 wolves left to complete the 10th quest. As we continue, we decide to clear the entire area before getting the final quest from that ghost near the burning forest. So we keep fighting and clearing the way. Ahhh, we got another wolf, one left to find until the 9th quest is complete. Clear out the worm on the hill and the skeletal rangers that spike you as you pass by......
We come up around the hill where there is the Mage NPC standing. We carefully pass by him, to finish off the 2nd wolf. Got him! There is a group to pull away from the patrol, got them too!! Finally, there is the patrol left, and then it should be clear to get the 11th quest. Now, keep in mind this whole time we have been very careful and this Mage standing alone has not moved this whole time.....Then all of the sudden, he takes off on his own into the patrol and kills himself, before we have time to realize that he even moved! FAILED!!!!!!! Wow, talk about soooo close. After about 5 hours, it is all over.
Nobody spoke for a few moments.. I think that tops the worst way to die for me. My team was amazing, and our leader was awesome!!!!!!

Running the Deep. Clear through to Scorpion aspect...all but one dies right as we kill the aspect, but a group of outcasts is still alive and patrolling right on top of everyone's body. only one left alive is an ele with a normal rez sig. I'm closest to him with a sunspear signet. I could run the other way, lose aggro and rebirth one of the monks and we would be good to go. He sits there right at the edge of aggro, starts typing that he doesn't want to rez me in agg...WTF TWO FLESH GOLEMS RUN RIGHT FOR HIM!!!

A nice near-miss in the vein of FoW clears: finished tower of strength, went into burning forest to kill the shard wolf then coming back through tower of strength area to finish off khobay and collect the reward...tower mage runs off into the Shadow army and wurm. Only our very own DarkGanni was there to notice it...the rest of us had to rush to save his tarded ass so we wouldn't wipe. It worked...and we got our reward just fine.

Another bad way to die? Getting to the very end of Gates of Madness, all we have to do is kill Shiro, and he pwns the entire group with Impossible Odds, and the only person left alive has no res.
