definitely gates of madness. It just took so long to get masters on it. I actually had trouble even after getting all the blessings to kill shiro. In the end I ended up flagging the heroes outside the shrines and kiting shiro until he eventually got killed by the heroes.

I absolutely HATE the Venta Cemetary in NF, its so long, and it takes so long if I want to get the masters.

On my first couple of try while following the story line these gave me trouble:
Prophecy:
Elona Reach. No max armor and no max henchies. Party wipes on the first couple of try
Faction
Gyala Hatchery. Reading wiki for this one is a must. On my first attempt i had all the turtles killed on the first wave.
Nightfall
Moddok Crevice. Mostly because of the enormous time laps between the things you have to do to get master and the advices given by Dunkoro. None of his suggestions comes soon enough to avoid ruining the possibilities of getting master without reading wiki or going over the mission at least a dozen time.
They might not be the hardest of all, but they are the one that did make me wanna kick someone/something real hard

mordok crevice is a pain to get the 2 runners at the same time,
grand court---getting masters is a pain, but just getting thur its pretty easy with again the right h/h set up.

Aurora Glade... I have always hated that mission. I remember hating the jungle missions originaly, but now that I am experienced it just a boring bunch of spider slaying.

The first real mission in Factions. The first time was pretty fun because I was actually interested in reading all of the text but after doing it so many times I just want to hurt Togo because HE. NEVER. STOPS. TALKING.

Doing Bloodstone Fen with H/H...having to run back to pick up the seed so the vine will grow (stupid me didn't bring a speed buff duh!) yea so I was kinda pissed and bored.
They should add the option of allowing heroes to carry stuff.

So far, my least favorite is the Frost Gate in Prophecies. It's not hard and it doesn't take longer than other missions, I just don't like it. Maybe it's the ending I don't like.
Vizunah Square is a close second. Poor design makes it boring and tedious. The chaos and overkill is amazing.

I was going to say Gyala Hatchery for being such a pain to get Master's on, but I remembered something.
Arborstone. It's not the toughest mission in the game, sure, but a lot of the little plot devices reek of "this was only put here to annoy you". Stuff falling directly on your head, for ... none-too-well explained reasons. The cathedral even "collapses" on you when you're standing under open air in one part. Go figure.

Prophecies-- Misson with Leah Stone as the bonus. Ugggggggggh. Sooooooooooooo Slooooooooowwwwwwwwwwwwwwww. I would rather suffer the fate of a Greek Tragic hero than do that again.
Factions: Minister Cho's Estate. Needs a "Togo off" toggle button. I rather liked Vizunah on my monk and rit--just managed carnage all the way around.

wasn't a big fan of the 2 Gate missions in NF, (Pain and Madness), but they were a nice challenge, and not terribly hard once i got the hang of it.
In prophecies, i didnt hate a single one. i loved tyria. same with factions, i could never hate cantha. i WILL say that having to re-do Vizunah Square a million times because you keep getting 1 minute away from masters got tiresome after a while.
and if EoTN counts, Warband Of Brothers, Finding The Bloodstone, Destructions Depths. first took WAY too long to do, second took WAY too long to get to, (on that note, add elusive golemancer to the above list. nice dungeon, liked the zelda-esque puzzles, but too faaaar awaaaaay from Rata Sum. not looking forward to running there 8 times for my handbooks), and the third had the most beastly creature i have ever seen. Disk Of Chaos FTL.

gonna throw in a few more. pretty well any "if such and such dies you all die" missions. oddly enough, i dont count factions at all, because all of the "protect" missions, the people you had to protect could handle themselves for more than 10 seconds and didnt leeroy an entire platoon of charr. wonder who im referring to. and gyala hatchery was fun for me, dont know why. a nice challenge, and watching a whole mass of kurzicks go BOOM made my day.

2) The bonus takes even longer. Additional aggravation points for the fact that Leah walks SO slowly inside the fortress.
3) It's got a Monk mursaat boss.
4) It's home to several elite skills that you can't get any place else, and that do not spawn every time.
5) The last room is a pain.
About the only thing the mission has going for it is the cinematic.

I didn't like The Dunes of Despair. I'm sure that there were other, harder missions and bonuses, but that one sticks out fondly (*cough*) because I tried that bonus at least 15 times before getting it.
