Anet does ban with evidence when people try by-passing the filter. GW is not a game for little kiddies, they also expect that players will not behave like little kiddies.

Just an observation, I meant no offense. Also does arenanet scan names independantly(a non /report situation)?

Offensive names versus offensive chat are two different things. There is a filter in name creation so that some of the more offensive names cannot get through (no matter if you have filter on or not).
Anet employees may see you themselves and choose to ban you. They play the game too. Other than that, I do believe /report is how they hear about offensive names. I doubt they waste the resources/time/money to independently scan names. (Let's hope they're doing that for bots instead).
And lol at your last question. Each word independently is harmless? That can be said for A LOT of things that are very offensive when put together.

Saying that something is @#$%, such as that quest was the easiest $#!^ ever, or @#$% I'm tired.
Sexually explicit, harmful, threatening, abusive, defamatory, obscene, hateful, or racially or ethnically offensive language, is completely different.
Edit: saying something sexually explicit, harmful, threatening, abusive, defamatory, obscene, hateful, or racially or ethnically offensive even with the chat filter will still get you a ban if reported with a scrn shot.

When you try use the character, it will ask you to change the character name.
