Dishonorable? You got the wrong guy!

Ugh.

I just started playing again after a long break, so I'm new to this whole "dishonorable" thing. A couple of questions to start:

1) Does someone have to report me, or do I get it if I logout during a match?
2) How many times do I need to logout or get reported to become "dishonorable"?

The reason I ask is that, a couple of times, I have logged out during a match. Here's the situation: Myself and two other teammates are dead, and our monk is running around like a jackass while the three opposing players try to catch and kill the little darling. The monk is doing absolutely no damage, and the opposing team is doing no damage to the monk. The three dead members resign, but there is over six minutes left in the match, and it is going nowhere.

If anybody is dishonorable, it's the monk and/or the people who report players in this situation. I have NEVER left a match that was still being actively contested, and I am a bit frustrated right now. Is there anything that can be done about this situation?

Can I report the *hilarious* teammate that is wasting everyone's time?

Anyone can be reported I believe, just gotta have the right reason.

Who would be griefing? The runner wasting everyones time, or me for reporting? Can you report griefing?
