The answer to your specific question:
You start off on the continent of Tyria if you create a Prophecies character, but if you create a character in Cantha or Elona, you will have to unlock travel to Tyria via the appropriate quest.
Factions characters wishing to travel to Tyria for the first time should seek out Guardsman Chienpo in Kaineng Center and take the "Chaos in Kryta" quest.
Nightfall characters wishing to travel to Tyria for the first time should seek out Deras Tenderlin in Kamadan, Jewel of Istan and take the "Terror in Tyria" quest.
You can travel from Elona to Tyria and from Tyria to Elona by talking to the Forgotten Guardian in The Arid Sea (Tyria) or in the Crystal Overlook (Elona). You must have already been to the Crystal Desert in Tyria to travel from Elona to Tyria in this way, and you must be able to survive the Desolation by being able to summon the Junundu if you wish to travel from Tyria to Elona.
Likewise, you can travel to Elona or Cantha from Tyria if you have their respective campaigns, Nightfall or Factions.
Tyrian characters may travel to Cantha using the quest "Mhenlo's Request" which is offered by Firstwatch Sergio in Lion's Arch.
Tyrian characters may travel to Elona using the quest “Sunspears in Kryta" which is offered by Lionguard Figo in Lion's Arch.

The specific map for Nightfall to Ascalon City is "AC to LA Trip Map". You will need to complete the Nightfall mission(s) that unlock you to Lion's Arch first. When I got to Ascalon City, it looked different from the Prophecies. Also, no one seemed to have any quests or missions for me and I just kept walking around and killing stuff as usual but without anything to gain.
Once you make it to Ascalon City, you can travel to Nightfall towns and Prophecies towns using the map like you always have except now, both campaigns are in the map instead of just one.

Once you have finished istan and are at docks, assuming you have profecies its a simple quest and you're in profecies.
However, you will arrive at Lions Arch which isn't the beginning of the game, so if you wish to play it through from the start, get a run (guildy, purchase or try it yourself) back to ascalon city and you're all good.

To condense what Usup said:
Finish "Time is Nigh"
Speak to Deras Tenderlin in Kam and take the quest "Terror in Tyria." Its a simple quest, no fighting involved.
And yes, ferry still works as long as you are partied with someone else doing that quest.

cosyfiep
there are 2 quests that you take to get to the other chapters...one is the one mentioned in the post directly above yours the other is http://wiki.guildwars.com/wiki/Plague_in_Cantha
you do one then the other (come back to nightfall and then take the other), after which you can travel to the others via M and clicking on the boat.

Same goes for Factions. Basically as soon as you can leave it's starter isle you can travel to the other 2 campaigns and Eye of the North too. For Eye you need to be level 10 but they is no big deal. For the other campaigns, no level requirement is even needed. Only Proph-born toons have a harder time usually needing a friend to run them to Lion's Arch since they enter the main game through Ascalon and LA is the portal city to and from Proph.
Go to wiki and google "campaign travel" and read all the details for yourself. The game was made to make it easy to switch around early on to all three campaigns and Eye.
