I would like to see a title that over-rides any other title, warning if a player has had any action taken against them in the past week. This follows the same reporting rules as exist already, but publicly shows it. Let it be their title for one week, to be worn in shame. Let them be harassed and ridiculed by their fellow players until they leave the game or adopt a more acceptable behavior.
This title would be 1 tier, not count as a max title, and would be removed one week after the date of offense. During that week, their emotes would be locked and the option to change titles would be removed. This title could also come with an effect added to annoy the offender. For instance, while under the effect they move 50% slower in towns and outposts, and merchants, the priest of Balthazar, skill trainers, Xunali agents etc. won't deal with them.

If serious, I suggest you find more constructive ideas to suggest. Although it may help people be better in terms of leeching, leaving, etc. The only possible way is if this is a 1 tier, non-maxable title. That way people wont WANT to do that stuff.

The pic is just a lame joke, I know. But the concept holds water. I would not make it anything people should aspire to, but rather a way of singling out the undesirable party members who are likely to work against your efforts.
Non-Maxable of course, and temporary. It should contribute nothing to them and serve only as a warning to the rest of us. There are subtler ways, I know. Like not allowing them to join parties. But shaming them is a very effective deterrent. The douche bag symbol is not a serious suggestion, as it would not effectively transcend the language barrier when playing in a global game. Still the use of symbolism may help.

Clearly none of you understand what the OP is suggesting. A lot of the people who do dishonourable things in-game regularly try to draw attention to their "achievements" by showing off high-end titles. With the new dishonourable system, players are discouraged from leeching or leaving, but it doesn't stop them parading around, playing their emote as much as possible while they wait for the hex to run out.
The OP is suggesting that instead their title be replaced by something else for a week, no-matter what title they try to display. It won't be a title track, it will just show other people that this person has a track record, and as such warns them against grouping with them. Perhaps as an addition, their emote also be replaced or removed.
I think this would be a good addition, as there is nothing worse than say paying for entrance into UW or FoW, only to have a monk or tank leave after 5 mins. Clearly the dishonesty hex thing doesn't work, perhaps this system would.

if it replaces the title, but is not a real title and removes the /rank emote.
Yeah, as stated... I like the public shaming idea, but people will farm this. Shame doesn't exist any more; it's an extinct emotion.
One way to make it less appealing to farm this title for lols would be to give it in-game effects, like the PvE titles. How about, hm... -1 to all attributes? All skills take twice as long to activate? Movement speed -25%? Something like that - not truly crippling, but annoying.
I'm thinking that would make the title less fun to wear, and also add to the offenders problems wrt finding groups...
