go to www.guildwars.com.

Click on Support from the nav bar
Click on your country from the list that is displayed (note, depending on your country, the support login link may not be required.)
Click on Support Login
Click the 'Ask a Question' Tab
Fill in the relevant fields
Click continue and submit.
