Well Enduring Harmony effect the chants/shouts that are already up on a character?

No, it will not affect anything before it is used. Or, put another way, it has no effect on anything already on the effects monitor.

In the same context, will it affecy the first occurance of a refrain or all follwing ones as well if they are maintained ?

Nope. All the Refrains are echoes. Enduring Harmony affects shouts and chants.
