Why am I Dying?

I havent played GW for over a year. I come back to go out and quest, and as soon as I leave the city, my health bar decreases until I die. What am i missing?

I am clueless. Please help!!

Is the weapon you are using Vampiric? If so, you will have -1 HP degen at all times while it is equiped.

You might of took this quest, if so you would have a -2 health degen till the quest is finished.
