Where do these drop, is it mainly HM solo dungeons?

I've found them to just be random drops.

Randomly, I've heard more people get them in NM then in HM (but that might just be BS)

Anywhere that drops max gear has a (rare)chance to drop req. 8 max.

I think everywhere. I heard ppl get it in both NM and HM.

Max req 8s existed before HM came out, so its not like they "mostly" come from HM.
