Lately i have noticed my chars not putting out damage equal to the weapon they have. Like example, my W/Mo has a max Axe and only puts out like 3-5 damage per slice or my monk who has a max green sceptor and only puts out like 7-10 damage.

I checked the req and im exceeding them by like 2-4 points.

are there any other factors i need to consider?

Level 5 character will do less damage to a level 20 monsters than they will to a level 5 monster. Level is part of the damage calculation, but also the armor and resistances. So a higher level monster will have high AL armor. Plus, if you are using physical damage, and hitting a warrior, they have extra armor against the physical damage. Use elemental damage and hit a Ranger, they have extra armor.

Like the others said, there are various resistances that come into play, especially at higher levels.

Aside from the level of your character and the opponents, the damage is calculated based on their Armor rating and various resistances. By example, most Skeletons are resistant to slashing damage (what most axes do), while most plants (Aloe Husk, Life Pod, Oakheart, etc) are more vulnerable to it.
