In any case, it needs more damage... attempting to stick with your original "extened conditions" theme I'd go with:
[pious assault][Reap Impurities][Virulence][Mystic Corruption][Heart of Fury][Grenth's fingers][Withering Aura][Resurrection Signet]

Heart of Fury->Grenth's fingers->enchanted haste->pious->Virulence->Reap and pray they don't have restore or mend condition

Disease isn't a great enough condition to use [Tainted Flesh]. You can easily simulate the same conjoined affect with [Veil of Thorns], which allows you to take [Wounding Strike].
Also, [mystic corruption] is nearly worthless when you can spam Wounding Strike to reapply it. Crippled condition will also be reapplied a lot. I would lose Mystic Corruption for another attack skill of sorts. You are a dervish, and you should be taking the scythe's insane damage dealing capabilities in main focus.

In pve it is an interesting concept and will work most places in normal mode with a decent hero setup, however most places in hard mode, you will simply be dead before you have a chance to put up your enchantments and apply weakness to enough foes to make a difference. If you can find a way to incorporate a lot of daze/blind into this build, it would be a lot more effective. As it is, this build dedicates itself to doing something that is best done in small pieces throughout a well balanced team build, for example weakness (1 or 2 slots on a necro bar), burning (1 slot on a fire ele bar), cripple (not that useful in pve due to the speed enemies die), disease and poison are also not very effective.
This entire bar is completely outclassed by a standard ss necro in every way. That being said, I think it needs a lot more improvement before you submit it to wiki as a viable build.

I would advise against using a slot for extending conditions at all then - most conditions already last 8-15 seconds - any monster should be well and truely dead by that time.
If you're running with competent guildies or hench/heroes the following will do:

This will give you targetted deep wound and cripple, nearby bleeding and weakness (plus whatever the bad guys throw at you) and adjacent fire without sacrificing too much dps.

you really don't need earth prayers at 16. a few seconds less on Veil of thorns shouldn't make much difference since most mobs should be dead within 15 seconds or so anyway and a loss of 4 points of damage reduction from armor of sanctity shouldn't be that tough for a hero monk. 13 EP is the breakpoint for 4 pips of regen for MR so i would stick with that and lose the superior rune. that alone will make up for the loss of reduction from AoS. also mysticism should always be a multiple of 3 because those numbers are the breakpoints for the energy and health gain from your primary attribute. with all the enchantments in this build, that could definitely come in handy.
i think you could still kill stuff faster with a standard dervish build using 3 scythe skills and an IAS but it may be fun to change it up every now and then.
my attribute spread for this particular build would be something like

not really sure if that spread is possible and don't want to take the time to look it up. but something like that.
