A while back when I did a couple of missions in GWEN and I forgot to get a Hero’s Handbook, I could get one later on and I was able to buy the missing pages to fill out the parts I had already finished.
Recently I went back to GWEN with my necro and saw that I had finished the whole Charr section. So she got a Hero’s Handbook from Kodan, and afterwards asked for the ‘Written Evidence’, but he did not offer the missing pages (nor did Tyr the Skaald or anyone else who dealt with books). I thought that this was because I had finished the whole Charr section already. So with that character, I did the whole Charr section again.
However, just today, I loaded in one of my tanks who had no book yet and saw that she had completed ‘Against the Charr’ and ‘Warband of Brothers,’ but that was it. So I went and got her a Hero’s Handbook, but once again, when asking about ‘Written Evidence’ no missing pages were offered. The book still indicated that this character has finished nothing and Kodan went right to the part of telling her that her book is hardly filled.
I really don’t want to do those quests again with this character. Therefore, I would like to know what I might have missed. I know I could get pages before. I just can’t seem to get them anymore.

It only does this when you have done the quest without a book in your inventory. Otherwise, you could just add the pages and and then have maxed rep titles within minutes.

That doesn't work anymore. Seems like you can just get the missing pages only once per account, only when you have done the quest with no handbook along with you.

I assume the first time, you did it once (before the fill in the blank pages was around), turned in the book you put them in before, then got a new book when this was implemented. Well, it only works once.
If you did the mission without a book in your inventory once. You can fill it in one book. You do the mission 3 times without talking to the guy, you can fill it in one book. You do it once, fill it, do it again, you can fill it again. But you cannot fill in countless books for doing a mission once. (Max would be two times, but that's only if you filled a book and turned it in before this was implemented, and the second time being after it was).

I have never had a full book that I have turned in. This particular account (as well as the one my necro is in) hasn't even finished GWEN with any character in the account. A third account that has finished GWEN is the one in which missing pages were awarded (before finishing GWEN, mind you).
The character in question now, my warrior, did not have a Hero's Handbook in her inventory then or ever before. I went to Kodan for the first time and asked for a Hero's Handbook. No character in this account has gotten very far in GWEN. I am fairly certain that no character in this account has asked for missing pages. As for the former account with the issue, my necro is the only character I really play in that account and she is the only character to have left the Eye of the North outpost.

You can only buy pages if you have a Handbook already (just not in your inventory) when you complete the quests. If a character does not own a book at all, any quests you do cannot be added.

This is strange, indeed. You said that you were doing the GWEN missions a while back. Is it possible that back then you put together a partially filled book and turned it in, or put the book in storage/on a mule? This would explain the fact that the NPC's will not fill in the pages, as you already had those pages filled into a book when you did the missions.

Also, I am pretty careful about my records. I know which items are where and which account stores what and I have written lists of exactly where each character is in every campaign and whether or not they received masters, which heroes they have, what mini pet they get to have, who has done chest runs, when their birthday is, who is available to take people to the docks, etc. I am not at all careless about things. I check and double check everything and am super vigilant.
Well since there seems to be no explainable solution, I think I just shouldn't talk to Kodan anymore. Maybe there is something wrong with him. I have had no problems when I received my Hero's Handbook from other book providers.

The ability to fill in pages of a handbook (or guide) was added in the Oct 12 2007 update, along with the improvement of rewards for turning in books.

It was a mechanism designed for people that didn't receive the newly increased rewards because they turned in books before the reward increase. It was designed as a once-only affair.
It was not to allow you to complete quests, and then go get a book.
While I have sympathy for your plight, I think the correct answer for the future is to ensure that you have a book prior to undertaking these quests, and as you say, keep it in your inventory. (And you will also save yourself 100 gp per page!)

buying pages is only possible with a dungeon book ( if you have done it on the character without bringing the book though
), not hero's handbook

I did do the missions, at least on my necromancer, way back during that time when I played my monk who did get the missing pages. The only difference is that I never played my necro for a long time and so I bought the book much later. Thus, she never got credit for the missions as my monk did.
On other characters since then, I also received missing pages, but my warrior did the missions some time ago and then did not buy a book until very recently. So it appears that I just waited too long to pick up a book for those characters. Therefore, I would like to know which date the offer stopped because it wasn't just a one time thing that lasted for just a week or so. It must have lasted for while and then stopped, at least according to the experience that I have had.
