i did the cathedral of flames dungeon already during the vanguard quest line, but i never got an update in my book. do i have to go back to the cathedral w/o the quest in order to get it in my book?

You have to complete Temple of the Damned if you want that dungeon to be counted as complete. You visit the dungeon during the quest line but it isn't the actual dungeon, the quest simply takes place in the same area.

There are a couple of missions that venture into dungeons, but since they're mishes not dungeons themselves they only go into the hero handbook. Here's a list:
Warband of Brothers--->Cathedral of Flames
Vision of the Raven Spirit--->Raven's Point
Finding the Bloodstone--->Bloodstone Caves
