PvE it's probably CoP and SF. Ursan is nerfed and nobody uses it anymore. And Sunspear Rebirth Signet/normal resses are better.
In PvP it's Reversal of Fortune. Every time I've seen a championship of some sort, RoF dominated over other skills by at least twice the amount of uses.

You're going to get meaningless responses until you define what you mean by "used". Clicked the most often? Brought on the most characters? PvE or PvP? If PvP, across all formats or did you have a specific format in mind? Does the number of players in each format at any given time matter to you?
Lots of people play AB and Aspenwood, and those players don't take Res Sig (unless they're bad), decreasing the frequency of "Res Sig" on bars across all PvP formats. Everyone in RA (should) have a Sig, and there's probably as many people there as any place, but other skills get USED in RA much more often than Res Sig.
Monk skills (and frequency of usage) vary by format.

Also, are we talking currently have the highest frequency of use, or historically? If its historically it would almost have to be a prophecy skill due to time and the larger number that played back then along with continued use.

Cap Sig is def not true, as it is used once per area, sometimes a couple times if you have multiple. But skills are used a multitude of times, WoH is probably quite high on the list, as it is a Proph skill, it has been in high level use in pve AND pvp for a long while
