Where did your survivor die?

I ask because a couple days ago I made a new monk to get Legendary Survivor on -- I don't want to farm or brawl, I'd rather beat my head against a wall.
So far I have sabway, Labyrinthine armor & I'm at Sunjiang, so I'll have 15k kurz most likely by the end of the day. I'm at 302,577 xp, been playing normally; haven't farmed at all, just done missions with sabway or 2-man discordway.
And that's definitely not to brag, cause I know most people are way more hardcore about this. I have 41 hours over the past 3 days on this character. Just some background info...
Everything's been fairly easy so far, haven't really had to zone out even.
Anyways, I'm just wondering where you lost your Survivor at. That way I can learn from your mistakes and not do that (err=07 aside).

My first survivor died at Unwaking Waters (Kurzick Side). It was a ritualist that was using the old Ritual Lord spirit spammer build. I died 2 seconds before Kun bit the dust. I would have gotten lvl 2 survivor if only I could have lived 3 more seconds (my exp passed the survivor 2 lvl with the masters bonus I got for the mission). I did not farm to get that far, just did every Canthan quest up to that point and capped all the Canthan elites available on the kurzick side.
My most recent died in Thunderhead Keep. 20 second lag spike hit and I found my corpse dead on the ground.

was a FRESH survivor, only lvl 3 (so it wasn't a huge loss or anything.)
I was getting a run from LA to Ascalon.... the ettins popped out of no where from behind in that one cave. oh well? lol

That's common actually... I know about that 'cause I've ran that a lot. If you're doing a survivor you gotta run yourself on another account (or find a understanding runner) and have the rest of the team as hench or heroes.

My spear assassin suffered her first (and only) death in the Charr region of EotN, where that devourer boss is. Can't think of its name at the moment. I had a brief interruption in concentration and that ended it. That was around 6 months ago or so, although she was already Legendary Survivor and close to 2 million exp. I was trying to set a record for 'longest surviving spear assassin'. Working on that record was the only thing keeping me active on GW. Now I rarely log on at all.
Had a paragon die in the Grand Court of Sebelkeh mission, dervish (indomitable survivor) against Kephket and crew in the Tyrian desert.

I lost mine usually due to lag so all over the place. Sometimes during the path to EoTN, other times in NF's later areas.

Oh, btw, doing Sepulchre of Dragrimmar dungeon with H+H for the first time (and not wiki'ing the final boss), also just 50k xp from Legendary Survivor, is very fun.
P.S. Yeah, I know there's no title for 2 mil, it's just for fun.

My runner from LA to Beacon asked if I was a survivor and loaded up 3 heros for me as a bodyguard group (Jora, Dunkoro, Tahlkora) and flagged them by where I stood. I was also wise to purchase Natural Stride, dodge, and whirling prior to the run to help protect me.

Here's my list. All of my characters in PvE are Survivor R3 (any failers are deleted and restarted), I'll list their first death as well, hope it helps.
1. Monk. Died at R2 (500K XP) in Nightfall, on the way to Gate of Desolation to some rock beetle boss.
2. Ele. Got R3 and died shortly after during Vizunah Square. From that day on, I learned to not trust human Monks (yeah had the title but still didn't want to die, and it was not my fault). x_x
3. Monk. Got R3, died at 1.8 M in FoW. Knocklocked by Abyssals, deserved to die because I didn't see the aggro, got too late to kite.
4. Paragon. Got R3, died at 2.2 M in hard mode Arborstone. Part of the team had wiped, I tried to be a hero and died to the Rainstone eles.
5. Monk. Died at 1.193 M XP during dwarven boxing. Disconnect, I was very mad at the moment, but it was not much of a loss because I was farming the title that time anyway.
6. Assassin. Got R3, died at 1.6 M during Nahpui Quarter mission. Again, don't trust human monks like you would trust hench/heroes. 100% sure it was not my fault.
My current Monk (remake of #5) has 3.2 M XP without deaths still. As you can see I like to keep my chars alive for as long as I can, it's pointless but I have fun with it. I guess it's a behavior I acquired from playing Diablo II in hardcore mode.

Third (and final) try died after already getting r3 to a bunch of ice imps in Mineral Springs (I was casing the joint, and they nuked my team with mind freeze + 20 maelstroms at once)

My first Survivor, a Rit, died well on his way due to rubberbanding. Tried kiting, noticed the damage still dropping all over me annnnnnd toast. This was doing the Wajjun Bazaar farm.
Lost another Monk and Rit on the Wajjun Bazaar farm, one due to overextending from heroes and another due to similar lag/rubberband based deaths.
Got a Monk to r3 (my only one) doing HFFF. After that I wanted to celebrate by getting owned, so I went to UW HM with no armor and Frenzy and took 2k+ damage for a splendid first death.
Lost a Ranger around 850k with one day to go. I had just bought all the Scrolls I'd need to finish in a couple hours, sat down with my snacks, kicked on a movie, and lagged out of my FIRST round of Kilroy. Yeah. Eff you.
Lost a Warrior (he was going to be my "just play" Survivor) on Tahnnakai Temple mission against Bound Teinai when my hero Monk decided it would be a bad idea to SoA against Sliver which triggered 10x or more a second due to minions. I kited out of it with ~100 health and he ran in and nailed me with Star Burst. I remember this one in slow motion, watching the enemy skill monitor slowly fill up with Star Burst, looking at my health and thinking, "Shit".
Only one maxed it, not going to try it again.

My first attempt to get a survivor was on my elememtalist. He died in Nightfall on the island. I forgot where but i was too close to the enemies and i died. After i died i was pissed, but then started a monk. He hasnt died....yet.

pugs are the biggest killer of survivors! all the times i lost survivor was when i joined a pug. it didnt matter where but how. only team up with people you know are experienced and wont lead you into a deathtrap.

I've lost so many survivors over the year but for the few memorable ones...
-Getting degened to death in AB while trying to get enough faction points for Befriending Luxon. I was running with like 50 hp and bleeding cond. I was just watching the screen hoping it'd go away and exited when I had 10 hp. Yes, very stupid.
-Fort Aspenwood. I was under the impression that I was supposed to score a victory before moving on. I'm happily crippling everyone in sight when this one sin teleports next to me and starts unleashing hell. I panic and exit. Yeah...
-The Norn Tournament thing. I was going up against the Bison guy. Everything stands still for a min then I d/c. Come back, /deaths, exit, delete, and recreate.
-By like 200 dmg spear chucked by Lieutenant Mahrik in 'A Land of Heroes' when I had a 55 armor on.
-The Norn bear. I'm like, 'wazzup,' and the thing goes 'rawr,' and I'm like, 'oh shit 200 dmg,' and the thing goes 'rawr,' and I'm like, 'o shit 300 dmg?!?'
-The raven spirit quest in EotN. Not sure what went wrong but I was fine one moment and on the floor two seconds later. I sat there dazed as my character got rezzed and the guy I was partying with urged me to rez him. I just left...
-Like 900k Survivor doing 'One Man's Dream,' which summons Djinn Overseer and a Ruby Djinn. I had no idea what was up and had the party in the standard 3-3-2 formation when I triggered it. 90 hours of work gone in 3 seconds. I came back with the survivor I have now when I had maybe 1.2 mil and easily beat it by splitting the party. Yes, I spat on its corpse when I was finished.

1st attempt at a Paragon for R3 Survivor ended in Kilroys dungeon due to lag at 1.2 Million XP. Tried again the same way died due to lag at 980k XP. VERY upset at that time After a few days decided to try again but this time did the HFFF way. Got it

My last Survivor that died was a level 19 Dervish that tried to do Chasing Zenmai. It was the first time I was trying the quest after the method of getting Zenmai had been changed. I was right next to the boss and he made swift work of my progress. I think I took a week or two off of GW after that and deleted her when I got back. Never tried for Survivor since.
Before that, I had a Survivor die at some low level when he was fighting in the starter island of Nightfall, and I didn't realize that there was a level ~11 mixed in with level ~6s that I figured our team could handle with no issue.

I've made over 40 attempts since this time last year for survivor, between several professions.
However much you hate lag spikes, I can guarantee you I hate them more.
Most of these were Canthan characters who died in the Echovald to Ele/Rit bosses and the like. Nightfall characters died towards the end of Kourna. Prophecies characters anywhere from Ascalon to the Desert. About 1/10 of these were the result of it being late, and over-aggroing, the rest were lag spikes.
I am still trying for Legendary Survivor, and I absolutely refuse to give up or lower myself to boxing or other gimmicks. Only thing I'll do is play the game, and go for Skill Hunter at the same time

My single most hated place in the game has to be the Zen Daijun mission.

The bridge, Rift, elevation damage bonus, double damage, Shelter and Union as only prots -> KAA-BOOM!
If they make it though that - they made it.

After reaching LS - they just get killed in some random places. I allow myself to mess up and they die.

Mine died in a 1v1 with my friend after about 4 attempts. I needed some "decent" way to die, and thought losing to a friend would be cool.

My survivor was my para. I didn't realise i'd got the title till it hit max. First death was in DoA at the hands of jadoth.
I also finnished fow, uw, urgoz, doa and all 3 campaigns before that death....And people still say paras suck

Arborstone for my monk - I don't know what happened, but Dunkoro stopped healing me and my own WoH was interrupted by the damn ceiling. I didn't care too much though, the survivor title was just a nice "if it happens, cool" thing.
