If I already have had Prophecies for a while, will there be any issue with buying the Platinum edition (Proph + Eotn) and adding the access key to my account? If it will just add Eotn with no problems, will there be extra character slots?

I don't think you get extra character slots with EOTN, since it's just an expansion, not a campaign, and you play it with your pre-existing characters.
That said, you can just get the Platinum Edition and add that key if you have Prophs already. That's what I did - it yelled at me a bit that I already had Prophs added to my account but it did indeed add EOTN.

I just tried the same thing and it said I needed to create a new account, and by every indication on the menu screen EotN is not installed. Am I missing something? Is there any indication on the character select screen or world map in-game that EotN is in fact installed?

Mind you, EoTN is an expansion, so you will not be able to create a character in it. You'll only be able to bring your existing chars there, provided they are at least lvl 10 and have reach your campaigns main port city.

Here's the exact message:
"You cannot add this access key to this Guild Wars account because you will already have the rights this key provides. Please click the 'cancel' button and create a new Guild Wars account."
Then it shows the access key that I input.

Good grief...just figured it out. I had to click the "add access key" button in the bottom right AGAIN to get to a confirmation page to add EotN. Friggin' retarded after it TELLS YOU to cancel and create a new account. Idiot developers.
