A guildie of mine, finished EON and turned in his heroes book. Then he took a new HM one to work on. Funny thing is he has not done any missions yet in HM for EON, EVER! Yet there are 4 or 5 entries already?

That is not a glitch. Once you have completed the quests and start a new book, the same 5 quests do not need to be repeated. So you only need to finish the other 11 quests to finish the book in either nm or hm.

That's how it's supposed to work. The missions already in the book are actually containers for the repeatable missions you can play.
So, for example: The Missing Vanguard is completed only once you've cleared Against the Charrs, Warband of Brothers and Assault on The Stonghold.
The Missing Vanguard will ALWAYS show as a page in EVERY consequent book after you've cleared it the first time.
